Medicare Now Covers GLP-1s for $50 a Month: Who Qualifies for the GLP-1 Bridge

The rule that just changed

For as long as GLP-1 medications have been around, Medicare had a hard line: it would cover them for diabetes, but not for weight loss. That left millions of people over 65 paying full price or going without.

That changed on July 1, 2026. A temporary Medicare program called the GLP-1 Bridge now covers certain weight-loss GLP-1s for a flat $50 a month. It runs through December 31, 2027.

If you or a parent are on Medicare and have been told "it's not covered," this is worth a fresh look.

Which medications are covered

  • Wegovy injection

  • Wegovy pill (oral semaglutide)

  • Zepbound KwikPen (only the KwikPen; single-dose vials and pens are not covered)

  • Foundayo (orforglipron) pill

Compounded GLP-1s are not covered.

Who qualifies

You'll generally need all of the following:

  1. Medicare Part D or a Medicare Advantage plan with drug coverage.

  2. Age 18 or older.

  3. A qualifying BMI and health profile:

    • BMI of 35 or higher, or

    • BMI of 30 or higher plus uncontrolled high blood pressure, a certain type of heart failure, or chronic kidney disease (stage 3a or higher), or

    • BMI of 27 or higher plus prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.

  4. No existing Part D coverage for a GLP-1. If you already qualify through Part D for type 2 diabetes, moderate to severe sleep apnea, or MASH (a fatty liver condition), you'd use that coverage instead.

One helpful detail: your BMI and conditions are generally reviewed as of when treatment started. So if you've already lost weight on treatment, that usually won't disqualify you. Your provider may need to document your starting numbers.

What it costs

The copay is $50 for a one-month supply, no matter your income. Two things to know:

  • That $50 doesn't count toward your Part D deductible or out-of-pocket cap.

  • Injection supplies aren't included.

For comparison, these medications can run anywhere from about $150 to over $1,000 a month without coverage, depending on the drug, form, and dose.

How to get it, step by step

There's no separate application. It runs through your doctor and pharmacy:

  1. Confirm you have Part D or a Medicare Advantage drug plan.

  2. Book a visit to talk about weight management with a provider.

  3. Your provider reviews your BMI, history, and qualifying conditions.

  4. They prescribe a covered medication and send it to your pharmacy.

  5. The pharmacy submits the claim. It's expected to be denied at first. That denial triggers the prior authorization.

  6. Your provider submits documentation showing you qualify.

  7. Once approved, you pick up your medication for $50.

The process typically takes a few days to a week. There's no formal appeal, but your provider can resubmit with more information.

Don't have Part D? Open enrollment is now

Medicare's annual open enrollment runs from October 15 to December 7. If you don't have Part D or a Medicare Advantage drug plan, this is generally your window to add one for 2027. Waiting may mean late-enrollment penalties. If you have both Medicare and Medicaid, check your state Medicaid coverage first, since it may cost even less.

Don't forget the part coverage doesn't pay for

A $50 prescription is a big win. But the medication was never the whole plan. Protecting muscle, eating enough protein, managing side effects, and knowing what to do when the scale stalls all matter. We covered this in The GLP-1 Support Gap.

That's where Total Well Connect comes in. Members get a private community of people on the same journey, live help calls, guides on GLP-1s and metabolic health, and support between doctor visits.

The bottom line

If you're on Medicare and living with obesity or a related condition, the GLP-1 Bridge may cut your cost to $50 a month through 2027. The first step is a provider who knows the process.
